Request medical records

Patients, parents and legal guardians:

To request your child’s medical records, you may:​

• Submit a signed authorization form via fax or bring it to any Kids Plus Pediatrics office. Healthcare providers can also request to have your child’s records sent directly to their office with a completed authorization form.​

• Use our ROI vendor, RecordQuest for full medical charts or additional documentation.​ Most requests are processed within 5–10 business days, though older records may take longer.

Third-party requestors:

All third-party requestors must submit an Authorization for Release of Protected Health Information (available in English and Spanish). For patients under the age of 18, this form includes the signature of a parent or legal representative. In some cases, additional documentation may be required.

Third-party requestors can use RecordQuest to fulfill their medical record request.

Basic records


Basic records like last office visit notes, lab results, immunizations, or school/physical forms may be available for pickup at your Kids Plus Pediatrics office. Many of these records are also available through the patient portal.

Entire chart


Requests for additional records or your child’s full medical chart must go through RecordQuest. For fee estimates and other details, please contact RecordQuest. Fees and processing time will vary depending on the request type and delivery method (digital vs. mailed).
